To create a plan of care we begin with a thorough assessment including the following factors:

  • Medical: Has every aspect of your medical condition been considered? Get a complete history including medications, care providers, diagnoses and prognoses.

  • Physical: Does the physical environment need adaptations? Are new living arrangements needed? Is help needed with activities of daily living? Would hiring a caregiver help? Is assistance needed getting groceries, medications or other supplies?

  • Psychological: Could life be more fulfilling or meaningful? Are there concerns with adapting to change? Depression? Anxiety? Loss?

  • Financial: Are finances a concern? Have finances been put in order to provide for loved ones? Is help needed to speak with a financial advisor or attorney?

  • Spiritual: Are there still meaningful connections to a faith community? Are there unmet spiritual needs?

  • Social: Is isolation a concern? Is help needed with transportation to appointments? Is there an adequate support system?

  • Cognitive: Are there problems with memory? Forgetfulness?

  • Advanced Planning: Is there a will in place? Is there legal documentation for Medical Power of Attorney? Has what matters most been considered? Have the loved ones been sufficiently informed?

What we do next is up to you. Do you need ongoing support?

Here are some of our services:

  • In-person or virtually: We can check in regularly to see how things are going. Continue to assess the environment and cognition as needs and functioning change.

  • Appointment Assistance: Help find needed appropriate medical professionals, make appointments and arrange transportation. Help manage appointments. Accompany client as needed.

  • Emotional Support: Ongoing counseling and support either in home or virtual. Continue to reassess how people are feeling and coping.

  • Caregiver Support: Help finding caregivers. Help with finding respite care. Assistance with finding alternate caregivers as needed.

  • Complete Advanced Care Documentation: Assistance with creating a Medical Power of Attorney and Living Will. Identifying and actualizing legacy wishes.

Whatever needs arise, we can be there for you.
